Your clinic keeps your medical record on CollegioDx, a secure system provided by Trustqed Social Private Limited. Your clinic is responsible for your record; CollegioDx processes it only on the clinic's instructions.
What is collected: your name, contact details, date of birth, symptoms, examination findings, diagnoses, prescriptions, test reports and bills.
Why: to give you care, keep your medical record, send you your prescriptions and reports, manage appointments and billing, and, only if you choose, link your record to your ABHA under ABDM.
De-identified statistics may be derived from records. They never identify you.
Your rights: you may ask the clinic to see, correct or erase your data, or withdraw consent, subject to the law on medical-record retention. If your clinic cannot resolve a concern, write to the Grievance Officer at support@trustqed.com. You may also complain to the Data Protection Board of India.
Not for emergencies: in an emergency call 108 or go to the nearest hospital.
